壞死性腸炎是一種高死亡率的新生兒疾病。整體來說早產兒的發病率高於足月出生的嬰兒。而科學家們一直努力的去解釋壞死性腸炎的發病過程與尋找治癒的方法。幸運的是,先前的研究裡告訴我們從人類母乳中分離出來的 Disialylacto-N-tetraose (DSLNT) 在壞死性腸炎中扮演了相當重要的角色,可以預防嬰兒感染並減少發病率。然而,將 DSLNT 從人類母乳裡分離出來應用到治療上是不可行的,因為只能從供體取得少量的聚醣。雖然 Geert-Jan Boons 教授與其團隊已報告了利用酵素合成的方式來合成 DSLNT 天然物,但是控制GlcNAc α2,6-sialylation 的關鍵酵素 (

背景根據最新衛生福利部107年癌症登記報告顯示,結腸直腸癌居於癌症發生率第二位和死亡率第三位,其中約有20%新診斷病人屬於轉移性結腸直腸癌,其5年存活率低於15%。依據目前文獻研究顯示,轉移性結腸直腸癌的第一線治療,建議以標靶治療搭配化學治療。而近幾年有幾篇針對第一線標靶治療直接比較的相關文獻,呈現不一致的結果,因此對於最佳標靶藥物目前尚未定案。本研究欲探討轉移性結腸直腸癌病人使用第一線標靶治療之存活狀況及其後續治療之癌症預後。方法本研究採用全人口回溯性世代研究設計,利用衛生福利部衛生福利資料科學中心之台灣癌症登記檔、死因統計檔和全民健康保險研究資料庫,擷取2011年1月1日至2017年12

月31日期間診斷為結腸直腸癌並使用第一線標靶藥物的病人。後續依據病人使用的標靶藥品,分成兩治療組: anti-EGFR therapy和anti-VEGF therapy;我們評估標靶治療的治療型態、後續治療、存活分析和再次治療時間。存活分析採用Kaplan-Meier法估計存活曲線,並使用Log-rank test檢定是否有顯著差異性。危險比(Hazards ratios, HRs)則利用Cox proportional hazards model計算。最後運用傾向分數(Propensity score) overlap weighting方法進一步平衡兩組的干擾因子。結果本研究納入9,04

0位結腸直腸癌的病人,其中7,551位使用anti-VEGF therapy和1,489位使用anti-EGFR therapy。利用overlap weighting方法加權過後,兩組病人的基線資料呈現良好的平衡。當一線治療失敗後,有5,578位病人接受後續治療,其中最常見是使用化學治療,其次才為標靶治療。一線標靶治療的存活分析結果顯示,anti-EGFR therapy組病人的整體存活期中位數為21.5月(95% Cl, 20.2-23.0),anti-VEGF therapy組病人為18.6月(95% Cl, 18.1-19.1),兩組危險比為0.889 (95% Cl, 0.806-0

.981; p=0.0188)。而在腫瘤相關存活期的結果顯示,anti-EGFR therapy組的中位數存活期為22.0月(95% Cl, 20.6-23.3),anti-VEGF therapy組病人為19.0月(95% Cl, 18.4-19.6),兩組危險比為0.895 (95% Cl, 0.810-0.988; p=0.0276)。然而於再次治療時間的結果中,anti-EGFR therapy組的中位數存活期為10.2月(95% Cl, 9.7-10.8),anti-VEGF therapy組病人為9.8月(95% Cl, 9.5-10.0),兩組危險比為0.917 (95% 95%

Cl, 0.839-1.001; p=0.0539)。後續治療的存活分析結果則顯示,無論病人使用anti-EGFR therapy或是anti-VEGF therapy當作第一線標靶治療,其後續治療均呈現類似的整體存活期和腫瘤相關存活期。結論本研究結果顯示,轉移性結腸直腸癌病人接受一線的anti-EGFR therapy會比anti-VEGF therapy還可以顯著改善整體存活期和腫瘤相關存活期。此外我們也觀察到病人使用一線的anti-EGFR therapy,接受再次治療時間有比anti-VEGF therapy較長的趨勢。而在一線治療失敗後,後續治療的療效分析顯示,各組均呈現類似的結果
